ESIC online application will be available at esic.in employer portal. Employer first needs to login to their ESIC employer portal with their user id and password.

Here under employee section there is an option called Register New IP under employee section. Here employer needs to check whether that particular employee is already registered in ESIC portal or not. If employee is already registered then click on Yes and enter his or her previous IP number. If they don’t have any IP number then click on No. Now ESIC portal application i.e ESIC nomination form 1 will be appears on screen. Here fill all the required details.

Note: Employee Name and Relation details of Insured person are can’t be modified online, so be careful in filling these details in ESIC online application.

After filling all the required details click on submit, then IP number of employee will be generated automatically. The ESI IP number of employee will be available in PDF file known as print counter foil, this will be used to get ESIC pehchan card later.
